Unfortunately, no, many insurance plans are requiring their own specific intake forms. We will make sure that you receive to appropriate form for your specific insurance carrier at the time of your first appointment.<\/p>\n

Please wear comfortable clothes and bring the doctor\u2019s prescription for Physical Therapy and your insurance card with you. There will some paperwork to fill out on your first appointment.<\/p>\n
Do you accept self pay patients?<\/b><\/p>\n
Yes, however we are Medicare certified and by law cannot discount fees below the current Medicare fee schedule. We will try to keep your out-of pocket costs as low as possible and will give you an estimate of costs prior to beginning treatment.<\/p>\n
How will I know what my insurance benefits are?<\/b><\/p>\n
We will call your insurance carrier for you and verify you specific plans benefits.<\/p>\n
Can my doctor make the referral directly to you?<\/b><\/p>\n
